Person Centred Transition What we do Our transition worker collaborates with young people between the ages of 14 and 16 years to develop their person centred plans. Using a person centred approach and working directly with the young person and their family, the transition worker gets to know each young person, finding out what is important to them, their support requirements, their aspirations and what they hope to achieve in key areas, such as employment, education, housing, health, social care, independence and friendships The information is gathered within a framework that is intended to also inform the support planning process when the young person turns 18 years. Listening to individuals and their families and reflecting their viewpoint is a key part of the process. Richmond is keen to see development in this area for its young people and their families, and while we are making positive strides in this direction, we realize there is still a lot we can learn. We are always looking for opportunities to engage with other boroughs and individuals and families who are at the forefront of making change happen. Person Centered Planning is a funded six-month project with the Transition Team in Richmond. At the end of the project a report based on what we have learnt and the trends we have witnessed working with families and young people will be written. We are already starting to see a common pattern emerge in areas that seem to elicit the most concern from families – e.g. housing, maintaining circles of support and friendships for the young people after they leave school, fostering and sustaining community links, and meaningful employment. Working one on one with young people and their families and hearing their viewpoint, we are beginning to get a sense of what is working, what needs careful thought and certain resources that are clearly in need of developing or overhauling. The ground reality, as we are all aware, is very different for our young people wishing to lead independent lives and having choice and opportunity. We hope that this project will bring organizations and individuals and families together to bring about a positive change in how transition happens in the borough.
