Transition Support Planning

Before your 18th birthday and your move into Adults Social Care, you should start your Transition Support Planning. In conjuction with your social worker, RUILS can help you do this.

Below is a guide that plans what shoud be happening in the year leading up to your 18th birthday.

 

Young person is assessed, RAS is identified.

 

Referral to Ruils who work with the young person, family, friends and school to produce a support plan.

 

Draft plan is sent to social worker for general agreement. This is an opportunity to identify any issues and raise any questions. During this period the plan is agreed and the DP Letter of agreement is signed and the financial assessment form is completed. If there is a query on any of these they can be picked up in advance.

 

The young person and family start the process of recruitment by registering as employers with the RUILS www.employ-a-pa.co.uk website and start learning about becoming an employer. Once confident they put together their own recruitment handbook. Send out invites to our RUILS ‘DP employer sessions’, and encourage people to build their own support networks using our ‘DP Pooling Network’ ‘peer support groups’ ‘buddy scheme’ and ‘getting a life’ project.

 

 Make a selection of suitable PA’s to interview using the RUILS www.find-a-pa.co.uk website and/or other advertising outlets. Interviewing dates are set up, followed by employment checks and references. The position is offered and start dates agreed.

 

DP Bank details form is completed. Young person and family plan the induction, sets up payroll agent and prepare the employment contract using the ‘employ-a-pa’ website and plan for any training for their PA. The DP account is credited with funds to cover employment insurance. The Employers liability insurance application is completed. This can be done on line or by phone.

 

 The PA starts work, induction, training and probation period starts. The Ruils support service contacts the young person and family to see how things are going and offers the necessary advice, information and support.
